HOW TO PAY IF AFFINITY THERAPY SERVICES IS NOT A PROVIDER FOR YOUR INSURANCE:
You should always call your medical insurance on your deductible and co-pay and possible need for Pre-Authorization for psychotherapy with a non-paneled provider (a therapist that's NOT contracted with your medical insurance company),. Sometimes your insurance company will tell you that your counselor needs to call in and ask for a 'Single Case Agreement' or an 'Ad Hoc Agreement.' If so, please contact your counselor at Affinity Therapy Services and they will assist you.
If your insurance does not appear on our list of insurance companies with which we are paneled, Affinity Therapy Services will provide you with a bill/invoice. Payment for each session is expected at the time of service. Billing can be generated on a monthly basis and sent to you via email.

When using medical insurance, clients need to consent to their insurer gaining access to some of their personal information. If you do not wish to use medical insurance, you may pay directly via credit card or Paypal.
